Participant Responsibilities

While serving on a Next Step Ministries Mission Trip, I will seek to demonstrate my love for Christ and for others by agreeing to the following:

I agree not to use tobacco products, alcoholic beverages or illegal drugs.

I agree not to engage in or promote the use of obscene language, dangerous behavior, or sexual, illicit or illegal activity.

I agree not to have possession of or use any fireworks, firearms, knives or other dangerous items.

I agree to abide by the Next Step Ministries dress code.

I understand that any schedule changes must be made by or approved by Next Step Ministries Staff. In consideration of others and in order to receive sufficient rest, I will respect the lights out time each night.

I agree to stay with the group at all times.

I agree not to go into the room of the opposite sex.

I understand that others' possessions must not be tampered with or taken.

I will do my best to keep all areas of the lodging facility clean.

I am excited to serve the Lord through a week of service.

Authorization for Treatment & Release of All Claims

I, the undersigned, do for myself (or for and on behalf of my child under 18 years of age) give permission for an attending physician or hospital to administer medical care if deemed necessary by Next Step Ministries' employees and the physician or hospital staff during the Next Step Ministries Mission Trip.

I, the undersigned, do for myself (or for and on behalf of my child under 18 years of age) hereby release from all claims and forever hold harmless the directors, officers, employees and agents of Next Step Ministries from any and all claims and demands for personal injury, sickness, and death, as well as property damage and expenses, of any nature incurred by myself (or my children under 18 years of age).

I also assume personal responsibility for all medical bills (for myself or my child under 18 years of age).

Further, should it be necessary for me or my child to return home due to disciplinary action, medical reasons or otherwise, I hereby assume responsibility for all transportation costs.

Model Release

I understand that Next Step Ministries uses photographs and video recordings of participants for promotional materials and media related productions.

I hereby give Next Step Ministries and its representatives and agents absolute permission to take and use such photographs and video recordings of participant for any purpose and media, and waive any propriety, personal or other right to the photographs or video recordings.

I also release and hold Next Step Ministries and its representatives and agents harmless from any and all claims of blurring, distortion or alteration of such photographs or video recordings, whether intentional or otherwise.
